Public Schools Needing to Order Driving Eligibility Certificates
If you are a public school needing to order more Driving Eligibility Certificates, please follow these directions. *
1. On official school DISTRICT letterhead, type your name, your position, school central office name, central office mailing address, your e-mail address, and your phone number.
2. Indicate the number of high schools in your LEA (local education agency), the current number of DECs you have on hand, the number of DECs you are requesting, and the date you are requesting the DECs be received by your LEA central office.
3. Fax the letter to: Kea Alexander, Program Assistant Healthy Schools NCDPI at:
919-807-4046.
We do not send Certificates to school sites, only to the central office. The request must come on official school letterhead paper.
If you do not receive your certificates within two weeks, please e-mail Kea Alexander at kalexander@dpi.state.nc.us
DPI provides certificates to public school agencies only.
IF YOU ARE A PRIVATE SCHOOL OR HOME SCHOOL, please contact the Division of Non-Public Education at: http://www.ncdnpe.orgto order Driver Eligibility Certifictes.
* Please note, in an attempt to direct funds toward Driver's Training, we are requesting LEAs utilize ALL DECs on hand before ordering more. The color of the DEC is of no consequence; the DECs do not expire. Please contact your Driver's Ed instructors, high schools, and neighboring central school offices to ensure all DECs have been used before ordering more. Thank you and we hope this small action helps to direct more funds towards Driver's Training in our public schools. (If an emergency supply is needed, for example 50 or so within one week, please indicate on the fax request form.)
