Crisis in the Kindergarten: Why Children Need to Play in School

Kindergarten Health Assessment
NC DPI/DHHS Kindergarten Health Assessment
Information from The Division of Public Health:
Kindergarten Health Assessment forms were shipped (by state courier) to all LEAs in February. Each LEA received the number of forms based on K enrollment. We also shipped to the local health departments at the same time, the same numbers.
There are no changes to this year's form....correct date in upper left hand corner is "1/08" which is useful as an identifier to make sure folks are using the right form.
To order additional forms, you can call the School Health Unit at the Division of Public Health office at 919.707.5671 and speak with Rebecca Bartz, the Office Assistant. Also...on the Healthy Schools website the form and parents instructions can be downloaded and printed. (http://www.nchealthyschools.org).
